Northeast Passage Release Waivers

PARTICIPATION RELEASE

In full recognition and understanding of the responsibilities, hazards, and dangers inherent in the participation in activities of the Northeast Passage Program of the University of New Hampshire, I do hereby:

  1. Agree that I/we (if participant is a minor or has a legal guardian) have read and/or listened to and understand the written and/or verbal information provided to me related to the Northeast Passage sports event (indicated below) in which I and/or minor participant will be participating.
  2. Acknowledge and fully understand that I and/or the minor participant, will be engaging in activities that involve risk of serious injury, including permanent disability, sickness, and disease (including but not limited to COVID-19), death, and property damages. Further, I/we understand that there may be unknown risks not reasonably foreseeable at this time.
  3. Assume all the foregoing risks and accept personal responsibility for any damages following such injury, permanent disability, death, or property damage.
  4. On behalf of myself and my spouse, children and/or heirs, I agree to release, discharge, hold harmless, and covenant not to sue the NORTHEAST PASSAGE Program, the University of New Hampshire, the University System of New Hampshire, and their trustees, officers, agents, coaches, volunteers, and employees from and against all claims, demands, actions and causes of action for damages which I and/or the minor participant may sustain or incur due to personal injury, illness, property damage, loss or death arising from participation in the activity, whether or not the result of negligent acts or omissions on the part of Northeast Passage Program or the University of New Hampshire.
  5. Indemnify and hold harmless the NORTHEAST PASSAGE Program, the University of New Hampshire, the University System of New Hampshire, and their trustees, officers, agents, coaches, volunteers and employees for any personal injury, illness, property damage, loss or death sustained by any third party arising from my and/or the minor participant’s involvement in this activity.
  6. In the event that my and/or minor participant’s involvement in the activity causes damage to the property of the University of New Hampshire, we further agree to indemnify the University for such loss.

PHOTO RELEASE

Northeast Passage uses photos and videos to tell our story. We treat all photos and videos with care, and we do not use full names or other personal information in our text or descriptions. Thank you for signing our photo release! 


On behalf of myself and/or the participant (for whom I have legal responsibility), I give Northeast Passage and the University of New Hampshire permission to record and use photographs, videos, or other visual likeness and voice recordings taken of me/the participant in Northeast Passage activities in all forms and media, including publications, catalogs, brochures, websites, exhibitions, videos, digital media, and social media (such as Facebook and Instagram), for the purpose of education, advertising, fundraising, communication, public relations, training or any other lawful purpose.  I understand that participation in the program/activity is not contingent upon the granting of permission to use an image or likeness in this manner. 


CONFIDENTIALITY STATEMENT 

  • I agree to respect confidentiality of information.  As an employee, volunteer, or student, I understand the importance of confidentiality and the integral effect this has on safety and the ability of Northeast Passage to best serve individuals. 
  • I agree to keep all participant information within the organization's confines and will only discuss it with staff, related faculty, other volunteers, and students if necessary and appropriate. 
  • I understand that I may have access to information that is not public knowledge, and agree not to disclose this information, unless the individual is thought to be at risk of harm to themselves or others or is behaving in an unlawful manner. 

CODE OF CONDUCT 

I agree to behave in a lawful, responsible, and safe manner, abiding by NEP Policies and supervision guidelines to maintain the safety of all and integrity of the organization.
